How to File a Board Complaint

Complaints must be submitted using the Council’s complaint form. The completed form can be emailed to:

How to File a Consumer Complaint

How to Request Records

If you’d like a copy of your records, you must make the request in writing.  You’ll be required to complete a Release of Information and submit a copy of your driver's license.  

Records will be made available within 15 calendar days.  

Please be aware that there’s a $40 fee for hard copies of records.  

An alternative to requesting the entire record is to request a summary of counseling services received.  There’s a $25 fee for a summary letter.
